Competent Adults
A competent patient is defined as one who has the mental capacity to consent to medical treatment, including understanding and retaining information relevant to the decision, weighing that information to make the decision, and communicating that decision. You cannot initiate a proxy request for another competent adult. The patient must invite you to their own account. Adults can invite other adults to have proxy access to their MyChart account.
The quickest and most convenient way to grant or gain access between adults is through a MyChart invitation. Each adult must have their own activated Spartanburg Regional MyChart account. Visit our MyChart Info page if you need help getting started.
If you would prefer a paper form, please note that you must complete the Proxy Invite Form and return it in person to your physician’s office. The patient wanting to grant access to their account must be present.

Power of Attorney / Caregiver
If you have power of attorney or the patient is incapacitated or incompetent, access can be requested using the Proxy Request Form. To gain access to the patient's account:
- Step 1: You must be 18 years of age or older and have an activated Spartanburg Regional MyChart account. Visit our MyChart Info page if you need help getting started.
- Step 2: Complete the Proxy Request Form by downloading it below.
- Step 3: Submit the form one of four ways:
- In person: Return form (with documentation) to the appropriate physician's office
- Fax: Fax form (with documentation) to (864) 560-9112
- Email: Scan and email form (with documentation) to [email protected]
- Mail: Return form (with documentation, if applicable) to:
