Collection Instructions

Collections require patient/specimen identification and labeling complete with blood bank armband number.   
Must be post reaction specimen.  Print FORM.TRAN.2047 from Lab Section of Enterprise Forms on Hospital Intranet and follow instructions.

Additional Information

Test performed stat.  Nurse to complete FORM.TRAN.2047 and send with specimen to transfusion service.  Collect urine and also send to lab labeled as post transfusion reaction.

Patient Preparation

Discontinue transfusion.  Follow instructions of FORM.TRAN.2047 & IM400.804.
