High School to Health Care Application

Before you begin your application, be sure to collect the following items:

  • A 250-word essay on why you wish to participate in the program
  • A copy of your high school transcript showing a 3.0 GPA or higher
  • A letter of recommendation from your high school principal, teacher or counselor

Applications are due Saturday, May 3, 2025.
