Medical Staff Application Request

 

The Corporate Medical Staff Office recognizes that the credentialing application process can be time-consuming for a provider.  To help alleviate delays, Spartanburg Regional Healthcare System (SRHS) utilizes the medical staff corporate office to process a Medical Staff or Allied Health Professional application for appointment to any SRHS facility. By utilizing this process, it reduces the amount of time required to process an application. The Medical Staff Services office utilizes one application, whether you are applying to one or more facilities or payor enrollment.

Minimum eligibility requirements:

  • Applicant must have a current, unrestricted license to practice in South Carolina or North Carolina, depending on practicing location, and have never had a license to practice revoked, suspended or limited by any state licensing agency within the previous five years.
  • Applicant must have a current, unrestricted DEA registration and a state-controlled substance license, when applicable, to practice patterns.
  • Applicant must have not had a DEA registration and/or a state-controlled substance license denied, revoked, or suspended within the previous five years.
  • Applicant must be located (office and residence) close enough to fulfill their medical staff responsibilities and to provide timely and continuous care for their patients in the hospital.
  • Applicant must have current, valid professional liability insurance coverage in a form and amounts satisfactory to the hospital.
  • Applicant must have not been, and are not currently, excluded or debarred from participation in Medicare, Medicaid, or other federal or state governmental healthcare programs.
  • Applicant must have not had a Medical Staff or Allied Health appointment or clinical privileges, scope of practice, or status as a participating provider denied, revoked, or terminated by any health plan or healthcare facility, including this hospital, for reasons related to clinical competence or professional conduct.
  • Applicant must not have resigned Medical Staff or Allied Health appointment, relinquished privileges or scope of practice during an investigation or in exchange for not conducting such an investigation at any healthcare facility, including this hospital.
  • Applicant must not currently be under any criminal investigation or indictment and have not, within the last ten years, been required to pay a civil money penalty for government fraud or program abuse or been convicted of, or entered a plea of guilty or no contest to, any felony; or to any misdemeanor relating to controlled substances, illegal drugs, insurance or health care fraud or abuse, child abuse, elder abuse, or violence.
  • Applicant must meet any current or future eligibility requirements that are applicable to the clinical privileges being sought.
  • Applicant must fulfill all responsibilities regarding emergency call based on specialty.
  • Applicant must have appropriate coverage arrangements.
  • Applicant must, if applying for privileges in a specialty that is covered by an exclusive contract, meet the specific requirements set forth in that contract.
  • Applicant must demonstrate recent clinical activity in their primary area of practice during the last two years.
  • Applicant must document compliance with all applicable training and/or educational protocols that may be adopted by the Medical Executive Committee and/or required by the Board or Trustees, including, but not limited to, those involving electronic medical records or patient safety.
  • Applicant must have successfully completed one of the following:
    • Residency or fellowship training program (in the specialty in which the applicant seeks clinical privileges) approved by one of the following:
      • Accreditation Council for Graduate Medical Education (“ACGME”)
      • American Osteopathic Association (“AOA”)
      • College of Family Physicians of Canada
      • Royal College of Physicians and Surgeons of Canada
    • Foreign medical graduates must complete three or more years post-graduate training by an AGGME, AOA, or Royal College and attended no more than two of the approved training programs.
    • Dental or an oral and maxillofacial surgery training program accredited by the Commission on Dental Accreditation of the American Dental Association (“ADA”)
    • Podiatric surgical residency program accredited by the Council on Podiatric Medical Education of the American Podiatric Medical Association
    • Satisfy the educational requirements as established by Medical Staff For Advanced Practice Clinicians and Allied Health
  • Applicant must be certified in their primary area of practice at the Hospital by the appropriate specialty/subspecialty boards as applicable:
    • American Board of Medical Specialties (“ABMS”)
    • AOA
    • College of Family Physicians of Canada
    • Royal College of Physicians and Surgeons of Canada
    • American Board of Oral and Maxillofacial Surgery
    • Commission on Dental Accreditation of the
    • American Dental Association (“ADA”)
    • Pediatric Dentistry Board
    • American Board of Podiatric Surgery (“ABFAS”)
    • Applicants who are not board-certified at the time of application, but who have completed their residency or fellowship training, must meet the certifying board eligibility requirements for their primary area of practice to be eligible for Medical Staff appointment.
  • Applicant must maintain board certification in primary specialty of practice on an ongoing basis.
  • Applicant must be a legal citizen of the United States or have a current legal permit to work in the United States.
  • For Advanced Practice Clinicians and Allied Health, applicant shall be certified by specialty board from the following:
    • American Academy of Nurse Practitioners (AANP)
    • American Nurses Credentialing Center (ANCC)
    • Pediatric Nursing Certification Board (PNCB)
    • National Certification Corporation for the Obstetric, Gynecologic and Neonatal Specialties (NCC)
    • American Association of Critical-Care Nurses (AACN)
    • National Commission on Certification of Physician Assistant (NCCPA)
    • American Midwifery Certification Board (AMCB)
    • National Board of Certification & Recertification for Nurse Anesthetists (NBCRNA)
    • National Commission for Certification of Anesthesiologist Assistant (NCCAA)
    • National Board of Surgical Technology and Surgical Assisting (NBSTSA)
    • Dental Assisting National Board (DANB)
  • For Advanced Practice Clinicians and Allied Health, applicant must have a supervising/collaborating physician and meet all applicable requirements of South Carolina or North Carolina law and hospital policies.
