If you are living with heart failure, education and special medical attention can keep you out of the hospital and help you live a more healthy, active life.
At our outpatient Heart Failure Clinics, you and your nurse practitioner work alongside your cardiologist, primary care physician and other healthcare providers to develop a plan of care to accomplish your medical and personal goals.
Our nurse practitioners are on-call 24 hours a day. Our team will begin working with you while you are still in the hospital. At any time, we can help you arrange emergency visits if you have new or worsening symptoms.
Our staff will also coordinate your care with home health or other agencies and make referrals to other specialists. We can help you get medications through one of several medication assistance programs, if you qualify. Our Community Paramedicine program is a community-based health approach offering follow-up care in the home after a hospital stay with heart failure.
Our Heart Failure clinics are located in Spartanburg, Greer, and Gaffney.
Program Eligibility
To be eligible for the program, patients must:
- Be diagnosed with heart failure
- Have a referral from a healthcare provider
Due to the complexity of chronic kidney disease, patients with a GFR<20 ml/min, or patients on dialysis, are most appropriately treated by nephrology.
