Spartanburg Regional Healthcare System’s Heart Failure Clinic Community Paramedicine program is a community-based health approach offering follow-up care in the home after a hospital stay with heart failure. A specially trained community paramedic uses virtual care to connect the patient to a Heart Failure Clinic provider in the comfort of their own home.
What to Expect
A community paramedic from the Heart Failure Clinic will call to confirm the previously scheduled appointment. Once the paramedic arrives at the home, the team member will:
- Offer telehealth assistance to connect with the Heart Failure Clinic nurse practitioner
- Assess your vital signs
- Review your medications
- Answer any questions you may have regarding heart failure
- Connect you with any resources you may need
- Communicate with your provider
- Perform in-home treatments (if needed) including, but not limited to:
- Lab work
- Medication administration
